At Forsyth Family Dental, we appreciate the opportunity to provide for your oral health needs. We stay abreast of cutting edge technology while providing our patients with comfortable, painless procedures. When visiting our office, you are greeted by our friendly receptionists in our waiting area.
General Information Request
Appointments
When you first arrive at the office please check in with the front desk. Help us stay on time by being prompt. We will do our best to stay on time, but we do admit that it is often impossible to predict the length of treatment time or what emergencies may have to be worked in. Please be patient. It may be you that needs emergency treatment someday. If you have to wait more than 15 minutes, please check at the front desk and they will find out the reason for the delay. We confirm our appointments by text or phone call in advance of your appointment, however, it is ultimately your responsibility to remember the date and time of your appointment.
Cancellations or Rescheduling
Please notify the office at least 24 hours in advance if you need to cancel or change your appointment. We may then use the slot for another patient. Those patients who fail to comply with this policy will be charged a $50 fee.
Insurance
Dental insurance is a contract between you and your insurance company to help you pay your dental bills. When you choose to see a dentist, it is your responsibility, and yours alone to pay any deductible, co-insurance, or other balance not paid by your insurance company. We will be happy to file your insurance for you after each visit and answer any questions you may have. We ask that you please furnish us with forms and all other information to file your insurance (for both primary and secondary if you are covered under two policies).
